I have my Meningitis Vaccine
Please be sure to submit a completed UTB Proof of Immunization Compliance form to the university through one of the following:
- Drop it by the Office of Student Engagement, Student Union 1.28, from the hours of 8am to 6pm, Monday through Friday.
- Drop it by the Tower at Main, from the hours of 8am to 5pm. Tuesday and Thursday they stay open until 7pm.
- Fax the form to 956-882-7154, Attention Heather Olague or email it to Orientation@utb.edu. Please call 956-882-5111 to confirm receipt.
- For your convenience, you may bring your vaccination record to the Student Health Services office who can verify your vaccine to complete your admissions application. A $5 administrative fee will be charged.

I do not have my Meningitis Vaccine
Any students entering in public, private or independent institutions of higher education must show evidence of having received an initial bacterial meningitis vaccination or booster during the five year period preceding.
If you need the vaccine the UTB Student Health Services Clinic offers the bacterial meningitis vaccine. If you are 18 years of age or younger, the cost of the vaccine is $15. If you are 19 years of age or older, the cost of the vaccine is $109. Student Health Services is located in Cortez Hall #237. No appointment needed, they take walk-ins.
